Here is my D100 Flesh Warping Effect Table. I would like to thank u/BarleyRegal for their help with coming up with ideas to my previous post.
45% mundane effects, 20% positive effects, 20% negative effects, and 15% both positive and negative effects.
Demon ichor is what remains behind after a demon dies. It’s a reduction of demonic blood, viscera, and bodily fluids with the consistency and odor of bile. Enough demons died in Avernus that their ichor forms pools and small lakes. Demon ichor have a unique property that when a creature comes in direct contact with it, it causes some sort of flesh warping curse.
Flesh Warping - Intense pain overwhelms you as you come into contact with demon ichor. You must make a Constitution saving throw (save DC 10) or one of the following effects occurs upon you; Fiends, oozes, plants, and undead automatically succeed on the saving throw and a remove curse spell can end one or all flesh warping effects.

Roll twice on the table. Reroll any further 1’s until you get different numbers.
The color of your hair, eyes, and skin becomes patterned of blue, red, and yellow.
Once in a while, you cough up a lump of hair that doesn’t belong to you or nearby creatures.
Your teeth retract into your gums and only ever extend when you have food in your mouth.
Your voice box disappears. You can no longer speak or cast spells that require a verbal component.
Your eyes push out of your head on the end of stalks.
Your eyes changed into compound arrangement (like a fly). You gain advantage on Wisdom (Perception) checks that involve sight.
Your lungs and stomach fuse together; Every breath that you take spits out a small amount of bile.
The skin, bones, and muscle of your check begins to crack and crumble away, creating holes that reveal the inside of your cheek.
All orifices on your body become lined with teeth.
Your nails grow into claws. The claws can be used as daggers. You are proficient with these claws.
You grow functional mouths on the palms of your hands. One is high pitched and the other is low pitched.
Your nose grows into an elephant trunk. You have advantage on grappling a creature made with using your trunk.
One of your hands becomes a crawling claw that is hostile towards you and attempts to sever itself from you.
You grow a set of cow-like udders from your stomach. Once a day they can be milked.
One of your legs grows longer than the other, reducing your walking speed by 10 feet.
Your jaw splits and twists against your face. Your mouth now opens vertically instead of horizontally.
Your voice box twists and burns. Whatever you say comes out in Abyssal, regardless of what language you attempt to use. You do not learn Abyssal from this effect.
Your skin becomes scaly. Once a day, you shed your skin into a vibrant new one.
Your fingers are now tongues. You taste everything that you touch.
A flap of skin appears over your ears. Whenever you attempt to lift the flap of skin, your ear canal illuminates a 15-foot cone of dim light.
Your bellybutton and your mouth swap places. You now talk through a tiny opening through your belly button and opening up your mouth reveals the inside of your stomach.
Your brain shrinks and smooths out. You have disadvantage on Intelligence checks.
Your nose grows in size whenever you attempt to lie. You gain disadvantage on Charisma (Deception) checks.
Your rib cage and spine become something to that of an exoskeleton, visibly tearing through your skin. Wearing no armor, you gain resistance against bludgeoning. slashing, and piercing damage.
A pair of wings, either feathered or leathery, sprout from your back, granting you a flying speed of 30 feet.
Your tongue becomes long, cylindrical, and tube-like akin to that of a bee.
Your entire vascular system becomes visible and far thicker than it had been. No longer blue, but instead a purplish color, ever pumping black blood through your blood.
You no longer perspire through your pores but instead vomit all of your sweat once every hour.
Your feet become cloven hooves and your knees reverse direction. The distance of your long or high jump is doubled.
Tiny wings appear upon your ears and fly away from you as they tear free from your head. You are now deafened.
You develop a second row of teeth, an independently controlled jaw within your jaw.
Every single joint in your body becomes loose and flexible. You have advantage on saving throws made against being grappled and restrained and Dexterity (Acrobatics) checks.
Your skin dissolves leaving bare muscle beneath. You have advantage on Charisma (Intimidation) checks.
Your torso becomes transparent, revealing the organs beneath.
Two of your upper and lower front teeth turn into abnormally large rat-like teeth.
Your body becomes unmoored to 3-dimensional space. Difficult terrain no longer affects you.
Your arms fused into your hips. You no longer have arms.
Every single hair on your body becomes dandelion, weed (not the drug, sorry to disappoint), and grass. Your body odor now smells like that of a wet lawn.
You split into two, down in the middle, stopping at your upper hips. Surprisingly, everything is still functional, half of your organs are on one side and your face and spine are on the other.
You grow small teeth all over your skin. You gain +1 to AC while not wearing medium or heavy armor.
You grow one arm from your chest. It acts independently from you and attempts to “liberate” the other arms from your body. You have disadvantage to all checks made with your arms.
A second face grows from the back of your head. The face acts independently from you and speaks only in Draconic. It can communicate with snakes and rambles about seeking vengeance against his phylactery, which took the shape of a human boy, identified with a lightning bolt shaped scar on his forehead.
Your eyelids fall off and no longer regrow back. Nearby creatures are creep out from your lack of eyelids.
Your pores begin to enlarge. Those around you develop a fear of Trypophobia.
Your arms and legs switch places. This switch prevents you from moving normally, your only option of movement is dropping prone and crawl.
A sticky mucus forms all over your skin. You have disadvantage on checks made against being grappled and restrained. You have advantage on checks against being disarmed.
An extra elbow forms in your forearm and an extra knee forms in your foreleg.
Your eyes melt away, leaving behind an empty eye socket that attracts Hellwasp. You are also blinded from this effect.
Clothing, armors, and objects that you are currently wearing, carrying, or holding, melds into your hands and body. You can no longer disrobe/doff or be disarmed, unless you or someone attempts to cut away the flesh that melded your stuff.
Your arms become tentacles with fingers on the ends. Your reach is increased by 5 feet.
The lower half of your jaw disappears, leaving your upper mouth exposed and your tongue hanging out.
Your organs begin to rot and wither away. You have disadvantage to Constitution saving throws.
Your shoulder blades erupt out of your back, growing and twisting into a spiraling shell.
Your spine bursts out of your back and forms jagged spikes along it.
Your legs grow incredibly long and springy. Your walking speed is increased by 10 feet.
Your head doubled in size, like a bobble head.
Your nose becomes hypersensitive. You gain advantage on Wisdom (Perception) checks that rely on smell but disadvantage on saving throws made against effects like inhaling poisonous gas through your nose.
Your ears become hypersensitive. You gain Blindsense up to 30 feet but disadvantage on saving throws against effects like thunderclap, and gain vulnerability against thunder damage.
You grow a newly deformed baby size body sticking out of your chest. It has the intelligence of an infant and it sometimes shrieks.
You grow a whip-like tail. The tail can be used as a whip. You are proficient with this tail.
You light up like a light-bulb, radiating 10 feet of bright light. You cannot be invisible and have disadvantage on Dexterity (Stealth) checks.
You grow another set of legs. As a bonus action, you can take the disengage or dash action.
Your head shrinks into your body and reappears at the front of your body. (search up blemmyes for reference)
You grow a set of antlers. They drop off every spring to regrow.
Your eyes turn black and are hypersensitive. You gain Darkvision, seeing out to a range of 120 feet but gain disadvantage to all checks that rely on sight under bright light, like the sun.
Your head, arms, and legs swap places, counter clockwise. (Your head is now where your left arm used to be, left arm on left leg, left leg on right leg, right leg on right arm, and right arm on head)
You grow a set of gills on your neck. You can now breathe underwater.
Your skin becomes dry and you can no longer produce sweat or shed tears. Each day, if you fail to immerse yourself in water for at least 1 hour, you suffer one level of exhaustion at the end of the day.
Your skin becomes bark-like. Wearing no armor, your natural AC is 13+DEX, but you gain vulnerability against fire damage.
You swell, tripling your weight.
An extra mouth opens up somewhere on your body. It acts independently from you and speaks in a condescending and petty attitude, waiting for the chance to out talk you, just to spite you.
You grow 1d4+1 bone spikes from the palm of one hand. You can fire these spikes from your hand as if using a hand crossbow. You are proficient with this weapon. You regain 1d4+1 bone spikes after finishing a long rest. You can no longer carry or hold anything with that hand.
Your leg begins to fuse against one another and morph into a crudely deformed mermaid tail. You no longer have a walking speed and must crawl on land but you have a (underwater included) swimming speed of 30 feet; Your Dash Action can be used for your swimming speed.
You grow a bulbous venom sac in your throat. You can cast the Poison Spray cantrip with Constitution as your Spellcasting Ability.
You become thin and skeletal, halving your weight.
Your dominant hand grows to four times its normal size. You have advantage on Charisma checks to attract a mate, provided that the mate has the preferences of a fiddler crab.
Twisted and gnarled branches grow from your shoulders. Whenever you sleep they take root in the ground and you must succeed on a DC 16 Strength check to be able to stand up afterwards. After finishing a long rest, the branches grow a single Goodberry.
You begin to feel as if bugs are crawling beneath your skin. Whenever your health drops below half of your maximum Hit Points, a swarm of insects erupts out from your skin. It is friendly towards you but hostile to your enemies and allies. They act on your initiative and always move towards and attack the nearest creature of you. This effect occurs again after finishing a long rest.
Tiny white wings appear on each side of your ankles. You have a hovering speed of 10 feet.
Your body becomes unusually brittle. You gain vulnerability to bludgeoning. slashing, and piercing damage.
You grow another head that acts independently from you. Its personality is the total opposite of yours and believes that you are the parasite that took over their body. However, they don’t have any control over your body.
The lower half of your body transforms into a Devil's Ride made of flesh and bone. You lose any movement speeds that you currently have, related to your lower body, and gain a walking speed of 60 feet. A seat-like space forms from your back where your butt used to be; A medium or smaller creature can take a ride upon you, like a steed.
All of your fingernails and toenails decays and falls off.
Both of your eyes morph together into one cyclops eye.
Your skin and stomach develops a resistance against harmful substances. You have resistance against poison damage and advantage on saving throws against effects like consuming poison or drinking alcohol.
Your body begins to rot like a corpse. You have advantage on Hide Actions by playing dead.
Your metabolism rate increases that negatively impact your energy and body temperature. You gain vulnerability to cold damage and during combat, every minute (10 rounds) you gain one level of exhaustion.
You develop an extra set of muscle along your voice box, granting the ability to amplify your voice. You can cast the Thunderclap cantrip with Constitution as your Spellcasting Ability.
You develop vulnerability against static electricity. You have vulnerability against lighting damage but you can cast the Shocking Grasp cantrip with Constitution as your Spellcasting Ability.
Your hair is replaced with feathers of murky water-like colors that appear all over your body.
Your bones along both of your arms begin to rapidly ossify, shooting your arms outward horizontally; Essentially, you are permanently t-posing.
Every joint in your body twists and coils up like a spring. You have disadvantage on saving throws against being frightened. When you are frightened, all of your joints spring outward like a jack in a box.
Any language that you hear can only be interpreted as Abyssal. You do not understand Abyssal with this effect; Abyssal sounds like harsh dog barking.
Your brain begins to swell, your forehead visibly swollen from your brain, and a large amount of white and grey matter spills out of your ears and eyes. You have advantage to Intelligence checks.
You develop a thick and luxurious layer of fur.
One of your arms detaches from you and drops onto the ground, dissolving into maggots, worms, and soil. You no longer have one of your arms.
The skin on your face becomes white as death. Your tongue extends to unnatural lengths. Geometric shapes in pallid black appear over your eyes.
A lump of flesh begins to grow on your back. After 1d6+1 days, the now enlarged lump bursts and out comes an exact naked copy of you. It shares all of your memories but absolutely believes that you are the copy, a Channeling or some sort of advanced Oblex. It attempts to convince your allies the same thing and is bent on replacing you.
You begin to suffocate as you feel a grip tightening around your neck. Eventually, pop, your head falls off. However, you are still alive. Your body and head can be separated within 120 feet, outside of that, you die. A simple piece of fabric like a red ribbon can easily reattach your head, as long as someone doesn't untie the knot of course.
